Preparing For Your First Appointment
Please print and complete Client Information and Health History forms and bring the completed forms with you to your first appointment.
What to Expect During Treatment
Diamondback PT For your initial visit, please arrive 10-15 minutes early and expect your treatment time to be anywhere from 1 hour to 1-1/2 hours.
Exercises will be progressive – beginning at an appropriate level depending on your condition. With increased strength and decreased symptoms, exercises will become more challenging. Your physical therapist will give you a home exercise/stretch program that will also be progressive. As you make improvements, your program will be adjusted depending upon your rehabilitation needs.
It is not uncommon to be sore after treatments, it is recommended that you keep your appointments so the therapist can evaluate your pain and adjust your treatment plan. Your treatment plan will be adjusted dependent on the feedback you give the therapist.
What to Wear For Your Appointment
- Neck Injuries – clothing with a loose neckline so that the site of pain is accessible.
- Shoulder Injuries – clothing with loose sleeves or tank tops so that the shoulder is accessible.
- Ankle or Knee Injuries – shorts, or pants that can easily be rolled to access the knee and approximately 6” above.
- Low Back Injuries – loose-fitting pants.
- Sports Injuries – athletic attire. Be ready to work! We want you back to play at a higher level than prior to injury!
Treatment Plan
Patient care in most cases, frequency and duration of your treatment is set by your doctor. Your physical therapist will complete the whole prescription from your doctor unless you and the therapist decide to discontinue, in which case your therapist will inform the doctor of reasons/factors for ceasing treatment.
Paperwork
Your insurance company requires you to fill out paperwork throughout the course of your treatment. Diamondback Physical Therapy uses the information that you provide to track your progress and provide feedback to insurance companies, attorneys, and workers' compensation companies. Thank you for your patience in this process.

Cost of Treatment
Due to differing insurance and payment options, the cost of treatment varies. Prior to your evaluation, Diamondback Physical Therapy will contact your insurance company for an estimated cost. You will be given that estimate in your new patient paperwork. Diamondback Physical Therapy also has cash rate options, accepts workers' compensation cases, liens and offers interest-free financing.
Cancellation Policy
Your appointment time is valuable and has been reserved specifically for you. If it is necessary to reschedule your appointment please provide us with 24-hour notice or a $25.00 cancellation fee will apply.
